Medication Management in Home Care

Course Description:
Effective medication management is a vital part of supporting the safety, wellbeing, and independence of people receiving care in the community. This course provides home care staff with a clear understanding of the importance of medication reviews in reducing risks, preventing medication-related harm, and improving health outcomes.
Course Aim: To equip home care staff with the knowledge and confidence to understand the purpose and benefits of medication reviews, recognise when they are needed, and support safe medication management practices in collaboration with the wider healthcare team.
Learning Outcomes:
After completing this course, participants should be able to:
- Explain why medication management is important
- Describe the responsibilities of RNs, ENs, and Clinical Care partners in medication management in home care
- Correctly administer and store medications
- Advise people on how to correctly dispose of medications
- Recognise and take action if medication incidents and errors happen
- Describe what a HMR is, when it is required, and the role you play in the process
